Information requested on the Intake form includes personal identifiers such as name, address, date of birth, telephone number, e-mail addresses, and a description of the issue for which the person wants assistance. This information is stored temporarily on the website’s database and is deleted once the information is retrieved by Disability Rights Ohio staff and put into our internal case management system. Only Disability Rights Ohio staff and its contracted website vendor have access to the information provided through our web Intake form. Information about a request for assistance that is received by Disability Rights Ohio or a request for Disability Rights Ohio advocacy is confidential as between attorney and client.
